Q: What does SSHP do?
A: SSHP provides support for those who are looking for help with housing. This includes, but is not limited to, case management services, financial assistance, voucher applications, transitional shelter, or help connecting with a landlord. Our programs are the following:
Intake Specialist - Our Intake Specialist is one of the first points of contact for SSHP. The Intake Specialist is able to provide a list of current available apartments, landlord contact information, and blank applications. The Intake Specialist will also complete any intake paperwork needed for coordination of services to assist in the households housing needs.
Coordinated Entry Case Manager - Coordinated Entry CM holds assessments for individuals or households who are at-risk of homelessness or those who are literally homeless. The purpose of this assessments is to evaluate the needs of the household, provide referrals to other agencies that the household feels is important, and evaluate eligibility for vouchers towards housing.
Motel Case Management - Case management for all those who are literally homeless residing within the motels located in Southern Windsor and Northern Windham counties. CM provides hygiene items, support to complete applications, provide available apartments listings, landlord contact information, and care coordination as well as any other support to help reduce barriers and increase income.
Case Management - Case Management is provided to support individuals that meet the HUD definitions of homelessness or at-risk criteria. Case Managers will also support and provide guidance to those who are part of our shelter program.
Youth Homelessness Demonstration Project Case Manager (YHDP CM) - CM for this program is focused on providing support for those ages 16-24 years of age. CM supports application completion, provides available apartment listings, landlord contact information, landlord mediation, care coordination as well as any other support to help reduce barriers and increase income.
HOP Financial Assistance Case Manager - The HOP FA CM provides support to those who are eligible for financial assistance through the Office of Economic Opportunity. To be eligible for assistance your household must be below the 50% area median income, show sustainability and demonstrate that, if not for this assistance, the individual would return to homelessness or continue to be without housing.
Department of Corrections Case Manager - The DOC CM supports clients being housed within our DOC shelter units through creating and supporting a housing plan, provides weekly case management, which includes budgeting, and assists in connecting individuals to resources that they have identified as important.
Landlord Liaison - Landlord outreach and education is an important component of programs that provide housing navigation and stability services. The Landlord Liaison is dedicated to landlord outreach to build relationships and point landlords towards resources and facilitate rental opportunities for households including providing housing navigation and retention support.
Shelter Program - We have 20 transitional shelter apartments varying in size. Thirteen of our units are in the town of Springfield, 2 in Chester, 1 in Windsor, and 1 located in Bellows Falls. Each of these units are funded through different agencies. Referrals are unique to each program.

A: Individuals and families experiencing homelessness or at risk of homelessness as defined by HUD. To participate in the agency's housing program the head of household participant must be at least 18 years of age. Individuals and families must be located within the northern Windham and southern Windsor counties, which include the following towns:
A: SSHP accepts referrals from client's case managers with a Release of Information (ROI) or the individual can make a self referral. All referrals should be directed to the agency's Intake Specialist. Past participants are welcome to reach out to their past case manager for future guidance.

- 4 General Assistance Pilot - Referrals to SSHP by Economic Services and Reach Up
- 7 HOP Units - Referrals to SSHP by Economic Services for those who are currently residing within the motels.
